| Question Number 1: I am seeking information relating to patients who died whilst receiving care from your mental health services, including inpatients- both informal and formal (sectioned) patients. Please provide the following information for each financial year (2025/26 to date, 2024/25, 2023/24, 2022/23, 2021/22, 2020/21): a) The total number of patients b) The total number of deaths c) The total number of unexpected deaths d) The total number of unexpected deaths that had a EUPD/ BPD (emotional unstable personality disorder/ borderline personality disorder) diagnosis e) The number of unexpected deaths that was classed as suicide f) The number of unexpected deaths that were not confirmed as suicides but were due to self-inflicted harm which led to the patient鈥檚 death g) The number of the unexpected deaths that progressed to a Serious Incident investigation h) The number of deaths that were referred (whether by the trust of by another organisation) to a coroner i) The number of deaths that resulted in a Prevention of Future Deaths (PFD) report being issued by a coroner *Wherever possible, please provide the information in Excel format. **If any of the above information is not held in the requested format, please provide it in the format in which it is held. | |
| Answer To Question 1: 麻豆国产 NHS Foundation Trust has responded to b,c and h. The data we have recorded is available from Apr 22 onwards. Also to note we are not a mental health trust therefore the ME office are unable to answer the other questions.
